A meeting chair change record should identify when responsibility passed between chairs and which business each person chaired. Preserve the attendance and agenda sequence before preparing the minutes. Recording what happened does not itself establish that the handover complied with the company’s constitution.

Establish the sequence
Keep the notice, agenda, attendance details and contemporaneous notes. Identify whether the original chair left the meeting, stepped aside for an item or remained present in another capacity.
Record the actual handover point using an agenda reference or an accurate time where available. Avoid suggesting that one person chaired the entire meeting simply because their name appears at the top of the template.
Ask the secretary to review the basis
Provide the applicable constitution and any relevant appointment or procedural records. The secretary should assess how the replacement was chosen and whether the record needs further clarification.
Separate chairing responsibility from attendance, voting and authority to approve a particular decision. This practical record does not decide quorum, casting-vote rights or the validity of a resolution.
Example: a different chair handles one item
The chair steps aside for a particular agenda item and resumes afterwards. Identify both transitions and the business handled in between. Do not erase the temporary chair from the record because the original chair later signs the completed minutes.
Preserve an accurate approval trail
Ask the appropriate reviewers to resolve uncertainties while the meeting is still fresh. Retain earlier notes and the agreed correction history. Keep the final record consistent with what actually occurred, and seek advice through advisers from associated law firms if a disputed procedural issue requires legal interpretation.
